Adult or Family Nurse Practitioner (ANP/FNP) – Outpatient Primary Care

Location: Fresh Meadows, Queens, New York
Employment Type: Full-Time
Schedule: Four- or five-day outpatient schedule
Compensation: $145,000–$170,000 annual base salary, plus potential quality and productivity incentive
Clinical Setting: Outpatient adult primary care

Join True Health NY

True Health NY is seeking an experienced, dependable and patient-centered Adult or Family Nurse Practitioner to join our Fresh Meadows primary-care practice in Queens.

Qualified candidates may include New York-certified:

  • Family Health Nurse Practitioners

  • Adult Health Nurse Practitioners

  • Nurse Practitioners with another New York adult-primary-care specialty certification appropriate to the assigned duties

The selected clinician will provide preventive, acute and chronic adult primary care, build and maintain a stable longitudinal patient panel and collaborate with the Fresh Meadows primary-care physician and integrated women’s-health team.

The Fresh Meadows clinic provides access to primary care, OB/GYN, women’s-health clinicians, ultrasound, diagnostic services and centralized administrative support.

This is a site-anchored Fresh Meadows position—not a multisite float assignment. The clinician will have a defined recurring schedule and clear responsibility for documentation, results, referrals, patient messages and continuity of care.

Position Highlights
  • Defined Fresh Meadows home office

  • Full-time four- or five-day schedule

  • No inpatient or overnight call

  • Established Queens demand for adult primary care

  • Opportunity to develop a longitudinal patient panel

  • Collaboration with a site-based primary-care physician

  • Dedicated medical-assistant and administrative support

  • Centralized scheduling, referrals, authorizations and billing support

  • Access to ultrasound and coordinated diagnostic services

  • eClinicalWorks electronic medical record

  • Guaranteed base salary with potential incentive opportunity

  • Potential sign-on assistance

  • Professional-development and future leadership opportunities

Clinical Responsibilities

The Nurse Practitioner will provide care consistent with the clinician’s New York certification, education, experience, demonstrated competency, payer credentialing and approved clinical scope.

Integrated Primary and Women’s Health

  • Address chronic conditions affecting reproductive health.

  • Support preconception medical optimization.

  • Coordinate postpartum return to primary care.

  • Improve preventive-screening and vaccination completion.
