True Health NY is seeking an experienced, dependable and patient-centered Adult or Family Nurse Practitioner to join our Fresh Meadows primary-care practice in Queens.

Nurse Practitioners with another New York adult-primary-care specialty certification appropriate to the assigned duties
The selected clinician will provide preventive, acute and chronic adult primary care, build and maintain a stable longitudinal patient panel and collaborate with the Fresh Meadows primary-care physician and integrated women’s-health team.
The Fresh Meadows clinic provides access to primary care, OB/GYN, women’s-health clinicians, ultrasound, diagnostic services and centralized administrative support.
This is a site-anchored Fresh Meadows position—not a multisite float assignment. The clinician will have a defined recurring schedule and clear responsibility for documentation, results, referrals, patient messages and continuity of care.

The Nurse Practitioner will provide care consistent with the clinician’s New York certification, education, experience, demonstrated competency, payer credentialing and approved clinical scope.
Responsibilities include:
New-patient primary-care examinations
Annual physicals and preventive visits
Medicare wellness services when qualified and credentialed
Same-day and acute-care visits
Evaluation and management of common adult illnesses
Longitudinal chronic-disease management
Medication reconciliation and medication management
Ordering and interpreting laboratory, imaging and diagnostic testing
Reviewing and communicating clinically significant results
Developing treatment, monitoring and follow-up plans
Coordinating specialist referrals
Providing post-hospital and emergency-department follow-up
Supporting preventive-care and population-health initiatives
Providing telehealth care when appropriate
Identifying patients requiring physician consultation or urgent escalation
Within certification and demonstrated competency, the clinician may manage:
Hypertension
Diabetes and prediabetes
Hyperlipidemia
Obesity and weight-related conditions
Thyroid disease
Asthma and common respiratory concerns
Common gastrointestinal conditions
Musculoskeletal complaints
Medication follow-up
Smoking cessation
Adult immunizations
Cardiovascular-risk reduction
Cancer-screening and preventive-care needs
Post-hospital follow-up
Other common adult primary-care conditions
The clinician will collaborate with primary-care, OB/GYN and women’s-health clinicians to:
Coordinate primary care for patients receiving gynecologic or prenatal services.
Address chronic conditions affecting reproductive health.
Support preconception medical optimization.
Coordinate postpartum return to primary care.
Improve preventive-screening and vaccination completion.
Reduce fragmented referrals and duplicative testing.
Maintain clear specialty boundaries and escalation pathways.
The Nurse Practitioner will:
Practice only within the New York specialty area in which the clinician is certified and registered.
Remain professionally responsible for diagnoses and treatment personally provided.
Consult the site physician when a condition is uncertain, higher acuity or outside demonstrated competency.
Escalate unstable or urgent conditions promptly.
Maintain written protocols and collaborative-practice documents when required.
Document significant consultations and referrals.
Avoid providing services outside the clinician’s certification, competency or credentialing.
True Health NY will establish appropriate practice documents after verifying the selected candidate’s certification and qualifying NP experience.
Documentation and Patient Follow-ThroughThe Nurse Practitioner will be expected to:
Complete charts within established practice timeframes.
Enter orders and charge-support information promptly.
Review and act on laboratory, imaging and diagnostic findings.
Communicate clinically significant results clearly.
Address refill requests and patient messages appropriately.
Complete referrals and follow-up orders.
Maintain an organized clinical inbox.
Close care loops involving abnormal findings and referrals.
Ensure appropriate handoff and cross-coverage.
Comply with clinical, payer, regulatory and organizational requirements.

Patient-volume expectations will be introduced through a structured onboarding ramp and adjusted for patient complexity, new-patient volume and clinical quality.
Required QualificationsActive and unrestricted New York RN license and registration
Active New York NP certificate and registration in Family Health, Adult Health or another adult-primary-care specialty appropriate to the duties
Current applicable national NP certification
Master’s degree or doctorate from an accredited NP program
Prescriptive authority or eligibility
DEA registration or eligibility
Current NPI
Current BLS certification
Eligibility for payer credentialing and malpractice coverage
Eligibility for participation in federal and state healthcare programs
At least two years of outpatient adult-primary-care NP experience preferred
Strong documentation and clinical follow-through
Ability to work consistently at the Fresh Meadows location
Three or more years of outpatient primary-care experience
Experience managing a longitudinal adult panel
Experience with Medicare, Medicaid and diverse community populations
Experience with chronic-disease and preventive-care programs
More than 3,600 qualifying NP practice hours

Strong recent references
Additional language skills relevant to the Queens patient population

Compensation and BenefitsTrue Health NY expects to offer an annual base salary of $145,000–$170,000.
The final salary will depend on certification, experience, scope, qualifying practice hours, credentialing readiness, references, schedule and internal equity.
The clinician may also be eligible for a written quality and productivity incentive after onboarding.
