Key Responsibilities
As a CNA with Dignity Home Health and Hospice, you will help patients receive appropriate personal care and support in their homes or other residential settings.
Your responsibilities will include:
  • Delivering personal care and CNA services to home health and hospice patients in accordance with your training, certification, and authorized scope of practice.
  • Completing eligible home health and hospice aide services in accordance with applicable Medicare requirements.
  • Helping patients with daily living activities and personal hygiene based on patient assessments, individualized care plans, and instructions from nursing leadership.
  • Providing patient care under the supervision and guidance of a Registered Nurse (RN).
  • Building respectful, professional relationships with patients, family members, physicians, and other members of the interdisciplinary healthcare team.
  • Observing patients for changes in condition, identifying potential concerns, and promptly communicating relevant information to the appropriate care team member.
  • Maintaining accurate, complete, and timely records of patient observations and services performed.
  • Using patient-care equipment safely and appropriately within your demonstrated competency and training, including wheelchairs, hospital beds, bed rails, oxygen equipment, and other relevant medical devices.
  • Following established care instructions, infection prevention measures, safety practices, and organizational policies.
  • Promoting patient comfort, dignity, and well-being through attentive and compassionate care.
Required Licenses and Certifications
Candidates must have the following current credentials:
  • Active Certified Nursing Assistant (CNA) certification in Utah
  • Current CPR certification
  • Valid Utah driver's license
  • Current automobile insurance
