Carroll Hospital is seeking a Discharge Support Specialist to support the Care Management Department through essential administrative and operational functions. This role plays a key part in meeting regulatory requirements, supporting patient throughput, and ensuring patients receive appropriate post-discharge services.
The Discharge Support Specialist serves as the administrative liaison between clinical departments, community resources, and third-party payers/insurance companies. The position ensures patients are informed of their rights and available choices throughout the discharge process, contributing to safe, timely, and effective transitions of care.
Key Responsibilities-
Provide administrative and operational support to the Care Management Department

Assist with patient throughput activities and discharge coordination

Support regulatory and documentation requirements

Communicate with clinical teams, community resources, and insurance providers

Ensure patients receive information regarding post-discharge services, rights, and options

Maintain accurate records using electronic medical record (EMR) systems

Utilize office equipment and Microsoft Office applications effectively
Education

High School Diploma (preferred)

Associate’s Degree (preferred)
Experience
-
Required:
-
1–3 years of clerical experience in a healthcare setting

Experience using Microsoft Office Suite and electronic medical record systems

Preferred:
-
Less than 1 year of experience in care coordination, care management, disease management, hospital/acute care settings, or customer service

Familiarity with medical terminology
