Description:

Under the clinical direction of the attending Physicians, and clinical supervision of the Psychiatrist independently assess, evaluates, treats, educates, and monitors individuals who are experiencing emotional and/or mental disorders as defined by the DSM-IV. Performs job duties that are consistent with a model that integrates primary mental health care with primary medical care. Demonstrates a high level of skills necessary to perform the job duties within regulatory requirements. Supports and follows through with the vision, mission, goals, and objectives of Family Healthcare of Hagerstown.

Demonstrates regard for, dignity toward, and respect for all clients, their families, guests, and representatives of other organizations to ensure a professional, courteous, caring, and responsive environment in the CHC.

  • Assists with questions or concerns to the patient's satisfaction.
  • Greets and establishes rapport with patients and their families, while obtaining patient history and assessment.
  • Maintains confidentiality of client and departmental information without any infractions.
  • Exhibits responsiveness to others in the organization at all levels to include returning phone calls, being punctual for meetings, and maintaining availability to meet.
  • Works effectively with external contacts, establishing positive relationships to the benefit of the Center.
  • Demonstrates awareness and sensitivity of cultural influences and diversity of patient population and attends cultural diversity training as offered.

Maintains respect for employees of the CHC and their individual commitments and contributions in support of the CHCs mission to provide consistent high quality patient services.

  • Demonstrates a willingness to work with and assist others to maintain productivity.
  • Consistently receives and gives suggestions and constructive criticism in a professional manner.
  • Demonstrates a positive safety attitude by keeping work area uncluttered and barrier free; responds to fire drills, disaster drills, and other emergencies 100% of the time; utilizes equipment in a safe manner.

Demonstrates efficient utilization of products, supplies, equipment, and time to promote financial viability through provision of services at a reasonable cost.

  • Employee understands goals and objectives of the CHC's philosophy.
  • Employee consistently practices efficient methods for completing work. Recommend changes in techniques and procedures to better accomplish assigned tasks.
  • As needed, willingly works overtime or adjusts personal schedule to accommodate the needs of the CHC. Consistently works to complete tasks in response to the needs of the CHC.

Ensures the Center’s operations are consistent with standards as identified in policies and procedures of FHH.

  • Appropriately responds in urgent/emergency medical situations in delivery of patient care.

  • Completes all Individual Treatment Plans (IT) within specified timeframes and ensures that all clinical documentation contains required elements in accordance with the FHH policy.
  • Continually develops professional skills as a Mental Health Therapist by attending programs or CME conferences.
  • Attends at least one program per year that relates to Integrated Mental Health Services.
  • Maintains licensure in the State of Maryland in good standing.
  • Participates in Quality Assurance program and patient/practice evaluation/surveys.
  • Suggests improvements in promotional materials.
  • Maintains a productivity rate of at least 65% for a given 12-month period.
  • When appropriate, consistently notifies patient’s PCP regarding their mental/social state to enhance their continuity of care and improve their overall medical/mental care.
  • In the setting of integrated mental health care, utilizes other support personnel, such as Mental Health Assistant, Case Manager, Health Services Coordinator, etc, effectively.
  • Independently maintains accurate and up to date caseload listing and is able to reduce frequency of therapy sessions or discharge patients, from treatment as appropriate.
  • Acts as a facilitator (or co-facilitator) in at least one group therapy a year.

Requirements:

POSITION QUALIFICATIONS:

Education:

Master’s degree in Social Work or counselor related field is required.

LCPC, LCSW-C, or LMFT preferred.

LCPC and LCSW-C Supervisor on staff.

Experience/Licensure:

Experience with pediatric patients and able to see kids as young as 4 years old and up is a plus.

Licensure/Certification within the State of Maryland and professional discipline at the level needed for third-party reimbursement.

CPR certified preferred.

Physical & Mental Effort:

Physical:

Minimum physical effort required. Intermittent sitting with freedom of movement. Occasional walking, bending, lifting, pulling, and reaching. Normal vision and hearing required.

Mental:

Frequent involvement in high stress situations. Must be able to remain calm under pressure, handle multiple stressors simultaneously, and maintain focus of treatment. Excellent coping and problem-solving skills required. Frequent periods of concentration, with attention to details with frequent opportunity for diversification of tasks.

Sensory requirements include the ability to articulate and comprehend the spoken English language, in addition to being able to read and write the English language.

Environmental and Working Conditions:

Work performed in a modern well-equipped environment. Interfaces with fellow employees, patients, family members, physicians, school officials, teachers, students and parents on a daily basis. Possible exposure to infection, virus and disease. Possible exposure to agitated and/or combative clients.
