Southern Highlands Community Mental Health Center is seeking a compassionate Case Manager/Care Coordinator II – School-Based to support youth and families within a school setting. This role focuses on coordinating care, providing support, and helping students succeed both emotionally and academically.

• Coordinate behavioral health, medical, and social services
• Assist with treatment planning under a licensed clinician
• Provide individual and group social-emotional learning (SEL) sessions
• Support students and families in crisis situations
• Collaborate with school staff, families, and providers
• Monitor progress and maintain documentation in the EHR

• Bachelor’s or Master’s degree in a related field (Social Work, Counseling, Psychology, etc.)
• Valid driver’s license
• Strong communication and organizational skills

• School-based or youth behavioral health experience
• Familiarity with CCBHC
• Licensure (LSW, RN, or similar)
