Are you passionate about supporting individuals as they transition from homelessness into permanent housing? Join Christie Ossington Neighbourhood Centre (CONC) as a Housing Follow-Up Support Worker and help people achieve long-term stability by providing mobile case management, life skills support, and connections to vital community resources. In this role, you’ll walk alongside clients as they rebuild their lives with dignity and independence.
What You’ll Do
- Complete client intakes and develop personalized support plans
- Conduct home visits and provide mobile case management
- Support life skills development (e.g., budgeting, housekeeping, tenant rights)
- Advocate for clients withlandlordsand service providers
- Provide referrals for health care, addiction treatment, and other community services
- Maintain thorough records and collaborate with internal teams
What You Bring
- Experience working with vulnerable populations, especially in housing or mental health settings
- Strong knowledge of harm reduction, trauma-informed care, and crisis intervention
- Understanding of housing systems, OW/ODSP, and Toronto-based resources
- Excellent communication, organizational, and problem-solving skills
- Training in non-violent crisis intervention (e.g., CPI) and First Aid
- Ability to work independently in the community and manage your own schedule
- Experience supporting Indigenous or marginalized communities is an asset
