We are seeking a dedicated and compassionate Case Manager to join our team at our residential facility. Key Responsibilities:
The Case Manager completes new client intakes as well as manages the client's case while in residential treatment. Some of these duties include facilitating weekly treatment team meetings, coordinating visits, scheduling, phone calls with parents/guardians, and more. Additionally, the Case Manager is the point person for communication with all involved with the case including the stakeholders, parents/guardians, insurance companies, and service providers. A driver's license is required for the Case Management position to travel to all the RTF facilities and school throughout their work week. This person will help coordinate all aftercare services prior to the client's discharge. Strong communication and organizational skills are necessary.
Skills and Qualifications:
- Bachelor's degree is preferred, along with case management experience.
- Strong interpersonal and communication skills
- Ability to handle sensitive situations with empathy and professionalism
- Excellent organizational and time-management skills
- Knowledge of community resources, healthcare systems, and relevant regulations
- Ability to work independently and as part of a team
- Valid driver’s license and reliable transportation may be required
- Typically, 1st Shift, must be flexible with hours
