Position & Responsibilities:
- Complies with accepted ethical conduct and Professional Standards of Nursing Practice as set forth by the American Nurses Association and the Nurse Practice Act of the State of Colorado.
- Practices and maintains patient safety and dignity by abiding by Federal, State, Local and Optimal policy & procedures as pertains to patient protected health information and HIPAA (Health Information Portability and Accountability Act).
- Provides direct skilled nursing care.
- Prepares equipment and materials for treatments, practicing aseptic/sterile techniques as required, and assists the client in learning appropriate self-care techniques.
- Administers medication as prescribed by the Physician and as permitted by Law and scope of practice.
- Assists in the evaluation of effectiveness of treatments and monitors client/patient's reactions.
- Contributes to the coordination of services in conjunction with other members of the health care team.
- Maintains accurate records.
- Reports significant findings or changes in the client/patient's condition to the Registered Nurse and client/patient's Physician.
- Records clinical/progress notes in client/patient's clinical record.
- Performs related and other duties as required.
- Complies with agency Policies and Procedures as well as Employee Handbook detailing Human Resources policies.
- Other duties / responsibilities that support the agency’s core values of Commitment, Advocacy, Respect, and Excellence.
- Completes emergency preparedness training as identified as appropriate for this employee level.
- Participates in exercises and drills for emergency preparedness, as required.
- Other duties and tasks as assigned by the Optimal Home Care Emergency Command Center communication tree in the event of a local, regional or national emergency and/or disaster.

- Preferably one (1) year of experience as a Licensed Practical Nurse.
- Has completed a practical (vocational) nursing program and is licensed in good standing in the state of Colorado and who furnishes services under the supervision of a qualified Registered Nurse.
- Must be able to communicate in the language of the client and document client related activity as appropriate
- Possesses adequate and effective written and verbal communication skills.
- Possess adequate and effective interpersonal skills.
- Able to lift 50 pounds on a regularly basis
