Licensed Practical Nurse (LPN)Job Summary

Under the direction of the RN Supervisor and/or Director of Nursing, the Licensed Practical Nurse (LPN) provides quality nursing care and support services to residents in accordance with established nursing standards, state and federal regulations, and facility policies and procedures. The LPN is responsible for delivering compassionate, resident-centered care while promoting the health, safety, and well-being of assigned residents.

Essential Duties and Responsibilities
  • Provide direct nursing care to residents according to individualized care plans.
  • Administer medications, treatments, and physician-ordered procedures accurately and safely.
  • Monitor residents' conditions and promptly report changes to the RN Supervisor, healthcare providers, and interdisciplinary team members.
  • Maintain accurate and timely documentation in accordance with regulatory and facility requirements.
  • Assist in developing and updating resident care plans.
  • Supervise and support Certified Nursing Assistants (CNAs) and other assigned staff.
  • Ensure compliance with all federal, state, and local regulations, as well as company policies and procedures.
  • Promote a safe, clean, and respectful environment for residents, families, and team members.
  • Participate in quality improvement initiatives and ongoing staff education programs.
QualificationsEducation
  • Graduate of an accredited Practical Nursing program.
Experience
  • Minimum of one (1) year of nursing experience required.
  • Previous experience in a Skilled Nursing Facility (SNF) or long-term care setting preferred.
Licensure and Certifications
  • Current, unrestricted Licensed Practical Nurse (LPN) license.
  • Current CPR/BLS certification.
Knowledge, Skills, and Abilities
  • Strong clinical assessment and nursing skills.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively as part of a multidisciplinary healthcare team.
  • Strong organizational and documentation skills.
