OUR MISSION
Wellness Equity Alliance (WEA) is a novel national public health organization comprised of a multidisciplinary team of population and public health experts with backgrounds in infectious disease, public health, emergency medicine, primary care, cardiology, pediatrics, psychiatry, community health work (CHW), nursing and advanced practice pharmacy. JOB DESCRIPTION
Purpose of the position
The Healthcare Practice Manager will lead day-to-day operations and performance across clinical and mobile healthcare programs in San Diego County, with a primary focus on supporting street medicine services serving vulnerable populations.
This role oversees operational implementation of grant-funded and contracted healthcare programs, ensuring services are delivered in alignment with program objectives, regulatory requirements, and community needs. The Practice Manager will play a key leadership role in launching and managing street medicine services that deliver care directly in community settings, including encampments, shelters, and other locations where individuals experiencing homelessness reside.
The ideal candidate is a dynamic, mission-aligned operations professional with experience managing complex healthcare programs, strong regulatory knowledge, and a passion for improving access to care for underserved communities.
In this role, you will collaborate closely with leadership, clinicians, outreach teams, and community partners to ensure operational excellence, maintain compliance with grant and contract requirements, support staff development, and strengthen partnerships with public health and community organizations.
Key Responsibilities
Operational Leadership
- Manage non-clinical operations across clinical and mobile care programs delivering community-based healthcare services.
- Lead operational launch and ongoing management of street medicine services serving unhoused populations across San Diego County.
- Ensure programs operate efficiently, safely, and in compliance with applicable laws, regulations, and contractual obligations.
- Develop and implement operational workflows and procedures tailored to community-based and mobile healthcare delivery.
- Coordinate supplies, equipment, scheduling, and vehicle or mobile unit logistics to support field-based care teams.
Grant & Contract Program Oversight
- Oversee operational implementation of grant-funded and contracted healthcare programs, ensuring services are delivered in accordance with program requirements and performance objectives.
- Monitor program deliverables, service targets, and reporting requirements tied to grants and contracts.
- Coordinate with leadership and program teams to ensure timely submission of required reports and documentation.
- Support program planning, launch activities, and operational readiness for new grant-funded initiatives.
- Collaborate with leadership to identify opportunities to expand services and strengthen program sustainability.
Program & Staff Management
- Supervise administrative personnel and support clinical leadership with scheduling, communication, and coordination.
- Provide operational leadership to mobile teams conducting street medicine outreach, ensuring staff safety, equipment readiness, and operational coordination.
- Onboard, train, and evaluate team members with a focus on trauma-informed and culturally responsive practices.
- Support interdisciplinary collaboration between clinical, outreach, and administrative staff.
Compliance & Quality Assurance
- Maintain compliance with local, state, and federal regulations (including HIPAA, OSHA, and applicable healthcare program requirements).
- Ensure operational alignment with applicable grant, contract, and programmatic requirements.
- Participate in audits, monitoring reviews, and contract compliance activities.
- Monitor key performance indicators (KPIs) and program metrics to support continuous quality improvement.
Stakeholder & Partnership Engagement
- Serve as an operational liaison to community organizations, public health partners, and other external stakeholders.
- Foster collaborative partnerships that support integrated care delivery across community-based settings.
- Represent the organization at partner meetings, community forums, and public health initiatives.
- Support coordination with local service providers to improve care access for individuals experiencing homelessness.
Financial & Operational Oversight
- Support program budget development and monitor operating costs across service areas.
- Collaborate with finance and billing teams to ensure accurate documentation, reimbursement processes, and revenue cycle alignment.
- Monitor operational resource utilization and identify opportunities to improve efficiency and sustainability.
Work Environment & Physical Demands
- Regular travel to sites, including outdoor/mobile environments for street medicine.
- Must be able to pass all required background checks, including those for correctional and educational site access.
Essential Skills and Qualifications:Required:
- Bachelor’s degree in Healthcare Administration, Public Health, Business, or related field (Master’s preferred).
- At least 5 years of progressive healthcare management experience across multi-site or multidisciplinary programs.
- Experience supporting care delivery in at least two of the following settings: correctional facilities, school-based clinics, street medicine/mobile outreach.
- Familiarity with operational and regulatory nuances of community-based healthcare models.
- Excellent organizational, communication, and team leadership skills.
- Willingness and ability to travel frequently within California.
Preferred:
- Background in public health, mobile healthcare operations, or correctional health services.
- Lived or professional experience working with people experiencing homelessness, youth, or justice-involved populations.
