Job Description

The Case Planner (PREIS) supports AFGs mission, and its goal to deliver quality case management services to participants in AFGs programs focused on youth development and risk prevention.

Key Responsibilities:

  • Perform community outreach. Travel to high schools to recruit and enroll young girls in the EmpowHer program.
  • Meet with participants and parents to answer questions about the program. Help participants fill-out an application and complete their enrollment packets, including signed consent forms.
  • Conduct two initial meetings with participants in the EmpowHer program, establish rapport and discuss their needs, barriers and goals. Create an action plan to achieve those goals.
  • Follow-up with program participants and their families to ensure their needs are being met and to provide them with additional resources, if necessary.
  • Act as an advocate for girls on school, community, interpersonal and family issues, as required.
  • Participate in case conferences to moderate and ensure client progress.
  • Maintain ongoing contact with participants, conduct home visits and make certain participants are involve in their support groups as planned. Complete progress reports.
  • Maintain participant records. Ensure documentation and data entry for all program activities.
  • Help the program meet its quality, scheduling and budget requirements.
  • Compile statistics and narratives that contribute to monthly, annual and ad hoc reports.
  • Develop relationships with other agencies/schools to ensure client access to needed services.
  • Perform other duties, as assigned

Demonstrated Abilities:

  • Solid organizational and case management skills
  • Ability to set priorities, take initiative and meet deadlines
  • Good communication skills, diplomacy and tact
  • Good decision-making, conflict-resolution and effective listening skills
  • Ability to handle crisis, cope with job stress and maintain confidentiality

Qualifications:

  • Requires an associate or bachelors degree in social work, counseling, psychology, sociology or criminal justice and a minimum of 3-5 years of related experience
  • Prior experience working with youth who are juvenile offenders, runaways, homeless or involved in high-risk activities is strongly preferred
  • Must possess a current drivers license, a good driving record and reliability transportation
  • Requires basic computer software and Internet skills
  • Bilingual in English and Spanish is preferred
