Family Triage Case Manager
Temporary Position: November 15, 2026 to April 15, 2027
Company Overview & Mission
Columbus House opened its doors in 1982 to provide services for men and women at least 18 years of age. Our goals quickly broadened from the mere provision of food and shelter and simple survival, to understanding and working toward overcoming the problems which cause people to become homeless.
Columbus House, along with its core of loyal supporters, friends, and volunteers, has remained committed to fulfilling its mission: "To serve people experiencing homelessness or at imminent risk by providing life-saving outreach, shelter and housing and by fostering their personal growth and independence. We advocate for and create affordable housing to end homelessness in our communities." ObjectiveUnder the direction of the Manager of Emergency Service Case Management, the Family Triage Case Manager provides case management to homeless families (mothers and children), networks with the greater New Haven shelter system and mental health/substance abuse/social/medical/housing service systems and collaborates with clinicians and other service providers as well as other Columbus House Case Managers to empower Family Triage clients to end their homelessness.

Qualifications
- Bachelor’s degree in human services required with a minimum of two years’ experience working in human services or related fields, preferably.Excellent organizational and time management skills with a deadline-oriented mindsetStrong written and oral communication skills employing the English languageProficiency is Microsoft Office and other common desktop applicationsAble to effectively use the local network of substance abuse, mental health, and social services providersFamiliar with criminal justice and able to engage, manage, motivate, and supportFamiliar with mental health, substance abuse and dual diagnosis issues and group dynamicsComplete knowledge base of all aspects of the delivery of person-centered case managementAble to meet all documentation requirementsKnowledgeable about crisis intervention, boundaries, confidentiality, universal precautions, harm reduction, trauma informed care and motivational interviewingValid drivers’ license with a good driving history
- Provide linkages for clients who need social, mental health, educational, vocational, recreational, spiritual, and or substance abuse services Provide transportation to appointments for clients Coordinate and collaborate with other agencies and social service providersOversee intake, departures and daily roster and programs Attend daily Family morning call (virtual).Attend weekly and monthly agency, program and network meetings Monitor all cases assigned and complete all documentation/charting required by Columbus House and its relevant fundersMaintain and update all required charting and notesEnter and maintain all client charts accurately and timely into the Homeless Management Information System Database Case Worthy Other related duties assigned in support of a 20-client case load
