Position: Fee for Service Adult Care Manager
Service Unit Compensation: $40.00 per service unit, or $42.00 per service unit for Bilingual Care Managers
Training Compensation: $30.00 per hour for all required training
Hours: Up to 20 hours per week with day, evening and weekend hours available
Location: Travel throughout Suffolk County
Start Date: Immediate
The Association for Mental Health and Wellness is seeking passionate Fee for Service Adult Care Managers (ACM) to make a difference in the lives of adults who have experienced trauma, significant behavioral/physical health and/or substance abuse challenges. Assist with navigating complex system of care, build supportive relationships and developing individualized care plans to help people meet their overall health and wellness goals. The fee for service ACM total compensation per service unit includes travel time, required administrative tasks, and the anticipated time to complete the client services. Service delivery is community based throughout Suffolk County. We are seeking candidates that have business day availability.
- Coordinating and monitoring behavioral, physical health and social services based on client-driven needs, build person centered care plans and annual comprehensive assessment plans.
- Building and monitoring linkages with natural supports and formal community resources including hospitals, outpatient clinics, rehabilitation, self-help, and social service providers.
- Providing supportive, recovery and wellness-directed coaching and counseling in-person and via telephone communication.
- A bachelor’s degree in human service/health discipline and two years’ Care Management experience is preferred.
- Attention to detail to ensure accuracy in the consent and enrollment process.
- Excellent inter-personal communication including the ability to establish empathic, empowering relationships with people who have experienced psychiatric, substance abuse challenges, trauma as well as chronic physical health conditions.
- Strong communication skills and collaboration with professionals involved in the client’s care.
- Understanding of the key elements of recovery and wellness, including the role of family and self-help.
- Sound organizational skills including computer skills (agency uses electronic health records, multiple IT platforms).
- Basic knowledge of the public health/mental health system, public entitlement and social service programs.
- Candidate must be a reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.
- Strong writing and computer skills, as electronic health records are used for documentation. Basic knowledge in computer applications such as Microsoft Word, Excel, Outlook and EHR.
- Clean and Valid NYS Driver’s License.
- Insured, reliable NYS registered passenger vehicle.
