Are you passionate about helping people improve their health and quality of life? Pivotal Health is seeking a compassionate, patient-focused Patient Coordinator / Sleep Health Navigator to serve as a key partner in guiding patients through their sleep health journey.

In this role, you'll work closely with physicians, healthcare teams, patients, and caregivers to coordinate care, educate patients on sleep disorders, facilitate diagnostic testing, and support treatment adherence. What You'll DoPatient Care & Navigation
  • Guide patients through the diagnosis and treatment process for sleep disorders.
  • Coordinate home sleep apnea testing and in-center sleep studies.
  • Educate patients on sleep health, treatment options, and prescribed therapies.
  • Monitor patient compliance and treatment effectiveness.
  • Follow up with patients regarding physician orders and care plans.
  • Assist patients with portal enrollment and ongoing engagement.
  • Partner with healthcare providers to ensure seamless care transitions following hospital discharge.
Care Coordination
  • Collaborate with physicians, case managers, nurses, and other healthcare professionals.
  • Review daily patient admissions and identify eligible patients for services.
  • Maintain accurate records and documentation within electronic medical record systems.
  • Track patient outcomes, treatment progress, and satisfaction metrics.
  • Serve as a resource for patients and their families regarding sleep health and treatment plans.
Quality & Program Support
  • Support quality improvement initiatives and patient satisfaction goals.
  • Maintain compliance with company policies and healthcare regulations.
  • Participate in professional development and ongoing training.
  • Contribute to the continued growth and success of our sleep health programs.
What We're Looking ForRequired Qualifications
  • Experience in healthcare, patient coordination, case management, sleep medicine, respiratory care, or a related clinical field.
  • Strong interpersonal and communication skills.
  • Ability to educate and motivate patients regarding treatment adherence.
  • Experience working with electronic medical records (EMR/EHR) systems.
  • Strong organizational skills with exceptional attention to detail.
  • Ability to manage multiple priorities while maintaining a high level of patient service.
Preferred Qualifications
  • Experience working with sleep disorder patients or sleep testing programs.
  • Knowledge of sleep apnea, PAP therapy, and sleep health best practices.
  • Background in patient education, care navigation, or healthcare coordination.
  • Clinical certification or related healthcare credentials preferred.

Physical Requirements
  • Ability to sit, stand, walk, and use standard office equipment throughout the workday.
  • Ability to occasionally lift up to 25 pounds and, on occasion, up to 50 pounds.
