Health Services Manager
$80,000-$90,000 per year, with credit for experience
Full-time, 40 hours per week, as scheduled

What You'll Do:
- Coordinate and oversee incoming patient referrals and admissions for the Health Center
- Conduct admission assessments to evaluate clinical appropriateness and rehabilitation needs
- Collaborate with hospitals, physicians, discharge planners, and families to support smooth care transitions
- Manage admission documentation, insurance verification, and authorization processes
- Lead efforts to achieve Medicare and short-term rehabilitation census goals
- Monitor referral and admission trends to support planning and growth strategies
- Partner with teams to ensure high-quality resident care and efficient workflows
- Support compliance with CMS, IDPH, HIPAA, and other healthcare regulations
- Maintain positive relationships with referral sources and promote Wesley Willows programs and services
- Prepare reports and assist with quality improvement and QAPI initiatives
- Provide leadership and guidance to admissions and clinical support staff
What You'll Need:
- Active Illinois Registered Nurse (RN) or Licensed Practical Nurse (LPN) license required
- Minimum of 3 years of clinical hospital, post-acute, or long-term care experience required
- Prior supervisory or team leadership experience required
- Bachelor’s degree in Healthcare Administration, Nursing, Public Health, Business Administration, or related field required, or a combination of education and experience
- Knowledge of Medicare, Medicaid, managed care, and skilled nursing admission processes
- Strong understanding of healthcare regulations, admissions, and care coordination workflows
- Experience with electronic health records (EHR) systems; PointClickCare experience preferred
- Strong communication, organizational, and problem-solving skills
- Ability to analyze data and use metrics to support operational improvement and census growth
