Job Title: Telephone Triage Nurse

Department: Medical

Reports to: Clinical Director

Classification: Regular, Full-Time, Hourly, Non-Exempt

Work location: Onsite

Starting Salary Range: LPN $56,124 - $66,028

Starting Salary Range: RN $64,624 - $76,028

Job Summary:

The Telephone Triage Nurse provides patient support through telephonic or electronic communication and assists in coordination of care through multiple service lines. This position prioritizes information as it is presented through a variety of sources.

Duties/Responsibilities:

  • Triage patients with both acute and chronic needs via telephone or other electronic communication and determine the next steps of action.
  • Documents encounters in the EHR in a timely manner to allow for follow up and gap closure.
  • Assist in coordinating care through multiple service lines and various departments.
  • Assist with a variety of paperwork involving care coordination.

Required Skills/Abilities:

  • Knowledge of standard nursing procedures and chronic condition education.
  • Strong customer service and the ability to maintain a positive attitude under pressure.
  • Ability to communicate effectively both oral/written with a broad base of stakeholders.
  • Attention to detail and a high level of accuracy and confidentiality.
  • Ability to think critically, solve problems, and work independently with minimal supervision.
  • Moderate level of desktop computer skills to include the O365 productivity software suite.
  • In-depth knowledge and efficiency with EHR systems.

Education and Experience:

  • A High School Diploma or GED is required.
  • Graduation from an accredited RN or LPN program is required.
  • Active RN or LPN license in the State of Tennessee is required.
  • BLS certification is required.
  • 1-3 years of experience. Outpatient experience is preferred.

Work Environment and Physical Demands:

Work is frequently active in nature and requires the ability to move around a large workspace on multiple floors. Requires some amount of computer work and hand dexterity and the ability to lift up to 10 lbs. on a regular basis.
