Position Summary

The Outpatient Team Lead – Advanced Practice Provider (APP) provides advanced cardiovascular and cardiology care to Midwest Cardiovascular Institute patients in the outpatient setting.

In collaboration with physicians and organizational leadership, the Outpatient Team Lead provides clinical and operational leadership to the outpatient APP team while maintaining direct patient care responsibilities. This role supports effective clinic operations, patient access, clinical quality, communication, and coordination of cardiovascular care across outpatient services.

Leadership Responsibilities

  • Provide day-to-day leadership, support, and clinical guidance to the outpatient APP team.
  • Support staffing, scheduling, coverage, and operational needs across outpatient services.
  • Serve as a liaison between APPs, cardiologists, clinical staff, administrative teams, and organizational leadership.
  • Support the onboarding, training, mentoring, and professional development of outpatient APP team members.
  • Provide coaching and performance feedback and communicate employee concerns to appropriate leadership.
  • Participate in team performance evaluations and development initiatives.
  • Identify opportunities to improve clinic workflows, patient access, communication, efficiency, and care coordination.
  • Support consistency and best practices across Midwest Cardiovascular Institute outpatient locations.
  • Participate in leadership meetings, quality improvement initiatives, and operational projects.
  • Assist with the development and implementation of policies, procedures, and workflows supporting outpatient APP services.

Clinical Responsibilities

  • Evaluate, diagnose, treat, and manage Midwest Cardiovascular Institute cardiovascular and cardiology patients in the outpatient setting.
  • Perform patient assessments, histories, and physical examinations.
  • Order and interpret diagnostic testing within the APP's scope of practice.
  • Develop and manage appropriate treatment plans in collaboration with physicians and the interdisciplinary care team.
  • Manage cardiovascular conditions within the APP's clinical expertise.
  • Coordinate referrals, follow-up care, diagnostic testing, and interdisciplinary services.
  • Provide patient and family education.
  • Maintain accurate and timely clinical documentation.
  • Provide clinical support across Midwest Cardiovascular Institute outpatient locations as needed.
  • Participate in outpatient call coverage as required.

Compensation: Pay starting at $120,000

Requirements:

Qualifications

  • Graduate of an accredited Advanced Practice Registered Nurse or Physician Assistant program.
  • Current and unrestricted Illinois professional licensure appropriate to the APP role.
  • Current national certification appropriate to the role.
  • Current BLS and ACLS certification.
  • Cardiovascular and/or outpatient clinical experience preferred.
  • Previous leadership, mentoring, supervisory, or team lead experience preferred.
  • Strong clinical judgment, leadership, communication, and organizational skills.
