Citizens Memorial Healthcare Foundation (CMH) is recruiting a Social Worker.

Lake Stockton Healthcare Facility (LSHCF) is a 90-bed long-term skilled nursing facility. A 30-bed memory care unit meets the specific needs of individuals with Alzheimer's disease and other forms of dementia.

A typical day will include: Develops brief psychosocial assessments, which examine the current problem(s) as well as relevant biopsychic-social and environmental factors. Communicates relevant aspects of the assessment to the patient, family, and members of the health care team. Develops and maintains a working relationship with community health, welfare, and social agencies, maintaining a list of community and government resources and agencies and meeting with those individuals when available in order to promote quality patient care. Responsible for the timely and accurate completion of psychosocial histories for each patient, interviewing patient and / or family and compiling the assessment within 72 hours of admission. Performs other duties as requested.

The position is Part Time, Days 9:00a.m-5:30p.m and some weekends
