Responsibilities:

• Visit patients in the home and assess their plan of care
• Submit patient records and documentation in a timely manner
• Play an integral role on the Interdisciplinary Care Team

Qualifications:

• Master's Degree in Nursing
• Bilingual in English and Spanish
• Licensed to practice in the state of California
• Licensed and insured driver with reliable transportation
• Understanding and acceptance of hospice principles of care
• Comprehensive knowledge and well-developed skills in medicine, pharmacology, pain and symptom control, and patient admissions criteria
