In our Psychiatric Residential Treatment Facility (PRTF), this role supports adolescents requiring intensive, trauma-informed behavioral health care, making strong clinical judgment, intake assessment capability, and discharge planning essential.

As our Admissions and Discharge Coordinator, youll play a vital role in welcoming individuals served into our programs and ensuring their transition is seamless from admission through discharge.
What Youll Do Receive, review, and organize all referrals and supporting documents for individuals seeking admission. Coordinate admission and discharge processes with physicians, community referral sources, and facility staff.Conduct intake assessments when licensed and credentialed at the Masters level, ensuring clinical appropriateness and documentation compliance.Manage discharge planning in collaboration with clinical and operational teams to ensure continuity of care and appropriate aftercare arrangements.Obtain, track, and maintain Medicaid and payer authorizations for admissions and continued stay, ensuring timely and accurate approvals.Maintain clear, ongoing communication with referral partners to ensure timely and accurate information sharing. Track occupancy, discharges, and reconcile bed counts to maintain accurate facility census data. Provide reporting on admission and discharge activity to Administration. Uphold compliance standards across risk management, infection control, and safety protocols. What Were Looking ForEducation & Licensure- High school diploma or equivalent (required)
- Bachelors degree strongly preferred in Social Work, Psychology, or related human services field or active Registered Nurse (RN) license; Masters degree in Social Work, Counseling, Psychology, or related field (preferred) for candidates completing intake assessments
Experience
- Two (2) years of admissions, intake, discharge planning, or care coordination experience in a psychiatric, behavioral health, or healthcare setting (preferred)
- Experience with Medicaid authorizations and payer processes strongly preferred
