SpiriTrust Lutheran is now hiring a Full-Time Day Residential Nurse Navigator (LPN) position that will be split between the Village at Utz Terrace in Hanover, PA and the Village at Shrewsbury in Shrewsbury, PA!

Education/Training: Possess a current unencumbered LPN license in the Commonwealth of Pennsylvania; current CPR/AED certification (or obtaining CPR/AED certification within 30 days of starting the position).

Skill(s): Speak and understand English; proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communication skills; valid driver’s license.

Experience: A minimum of one (1) years’ experience in related positions normally required.

Skill(s): Responsible for performing a variety of duties to provide a wellness-driven program for Residential Living residents; achieving goals as established in the Community’s annual operating plan; coordinating work within the Community, reporting pertinent information to the immediate supervisor; responding to inquiries or requests for information.

Performs a variety of duties to provide a wellness-driven program for Residential Living residents as follows:

  • Performs initial pre-admission meeting of incoming Residential Living residents.

  • Meets with residents upon occupancy, annually, and as changes occur to coordinate referrals of Residential Living residents to SpiriTrust Lutheran and community agencies to promote independence and best outcomes

  • Completes necessary addendums as required for supplemental services to meet conditions of occupancy

  • Monitors that residents with supplemental service addendums are meeting contractual requirements and are able to reside safely in Residential Living. Reports any concerns to Executive Director and RL Resident Review

  • Plans and provides health related educational and wellness programs

  • Coordinates on-site health service clinics for Residential Living residents

  • Chairs RL Resident Review to assist resident and family to make decisions regarding appropriate level of care.

  • Assists with transition to new levels of care, in conjunction with Guest Services Coordinators, Director of Sales & Marketing, Retirement Counselor

  • Responds to calls from residents' families, and healthcare professionals.

  • Acts as resource for the Residential Living residents, serving as a communication bridge with external acute / sub-acute health-care providers when temporarily not residing in residential unit.
