Job Description

Manager of Social Work

SCOPE OF POSITION:

Reports to the Director of Case Management. The Manager of Social Work is responsible for assisting the Director of Case Management with the functions of medical social work. The Manager of Social Worker will help assess and evaluate the care/services rendered, staffing levels, competency and educational needs of the medical social workers and effectively coordinate and controls resources. The Manager will support the medical social workers in providing clinical social work services for patients throughout the acute care hospital setting. Primary responsibility is for the inpatient units. In partnership with the Case Manager, is responsible for arranging the transition of patients to extended care facilities, rehabilitation hospitals or other specialized facilities. Uses independent clinical judgment and works collaboratively with the interdisciplinary team. Communicates, as appropriate, the patient’s clinical condition and care needs. Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age-appropriate care to the patient population served. Professional knowledge and judgment will be utilized when guidelines are unavailable or inappropriate for a given situation. Promotes the profession of social work and professional practice through collegial support and interactions.

RESPONSIBILITIES:

  • Exceptional Patient Care: Responsible for developing an infrastructure for assessing,determining, and coordinating the discharge needs of patients to facilitate optimum outcomesand throughput.
  • Excellent Communicator: Establishes effective relationships with external agencies providing services to the organization (such as vendors, care providers, post-acute providers, physicianorganization, community agencies etc.)
  • Management Skills: Responsible for managing key performance indicators for hospital throughput. Identifies appropriate staffing levels, assignments, and deployment for the department.
  • Active Teamwork: Establishes tools, processes, and systems to support the Social Work process and optimize outcomes.
  • Participate in safety huddle.
  • Participate in process improvement.
  • Staff development/education
  • LLOS meetings
  • MDRs
  • After hours call as needed
  • Analyze data and trends
  • Interface with external partners and insurance in community
  • Performs all other duties as assigned.

REQUIREMENTS:

  • Must have basic computer skills and ability to access internet and other programs applicable to Waterbury
  • Hospital procedures.
  • Education: Master’s Degree in Social Work required.
  • License: License Clinical Social Worker (LCSW) from State of CT required · Three years case management experience in a hospital setting · Two years supervisory /leadership experience preferred · Ability to effectively interact with all levels of hospital, governmental and medical staff · Excellent written and verbal communication skills.
