Responsibilities
Hospice Social Worker Job Summary
- Responsible for provision of social work services to include crisis intervention, advance care planning conversations, resource referral and emotional support and/or counseling to patients and families as needed.
- Ability to work in coordination with other disciplines at part of team to address needs related to serious illnesses.
Qualifications
Hospice Social Worker Qualifications:
*Masters Degree in Social Work required. NC LCSW, OR LCSW Provisional license preferred.
*Must obtain provisional license after hire.
*At least 1 year experience in social work and knowledge of end-of-life issues/medical social work preferred.
*Reliable transportation and valid NC Drivers license required.

*Some travel locally will be required, generally within a 10-mile radius.
*This position does count toward clinical hours for a LCSW license.
