The Cumberland County Department of Social Services (DSS) and the Cumberland County Department of Public Health (CCDPH) have received funding to implement an initiative which focuses on primary prevention by utilizing a whole person/family approach and addressing social determinants of health (SDOH). The initiative includes multi-disciplinary Case Management and Care Coordination teams responsive to community referrals to provide care coordination and case management services to address social determinants of health, address social needs, improve health behaviors and health outcomes, improve health equity, and reduce health disparities, reduce preventable hospital stays, and prevent and reduce child abuse and neglect.
This position is a social work/case manager position that will be a part of a team working with children and families to provide education and resources to address socioeconomic and needs physical/mental health needs in an effort to prevent or reduce entry into the child welfare and/or juvenile delinquency systems.
Employee is subject to be called upon anytime to help the agency respond to a public health emergency which may include, but is not limited to, outbreak investigations, staffing mass dispensing or immunization clinics, providing disaster relief, or other incidents (man-made or natural and intentional or non-intentional) as they arise, or any public health response requested by the Health Director.

Duties for this position include, but are not limited to:
- Conduct assessment of the family’s needs with their participation.
- Identify and provide resources to family that address their needs.
- Assist family with navigating service providers and their requirements.
- Ability to work with people of all ages and backgrounds.
- Ensure barriers to participation in programs/services to meet needs are minimized. Willingness to conduct in home and virtual visits with all family members.
- Complete written documentation of progress toward identified needs.
- Advocate for family members with service providers to insure successful outcomes.
- Develop positive working relationships with service providers.
- Participate in regular supervisory conferences.
- Collaboration with nutritionist, nurse, community social services assistant, supervisors and project managers in meeting program goals.

- Bachelor's degree in social work from an accredited school of social work;
- Bachelor's degree in a human services field from an accredited college or university and one year directly related experience;
- Bachelor's degree from an accredited college or university and two years directly related experience.
Directly Related Experience is defined as Human Service experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.
Applicants who have obtained their education outside of the U.S. and its territories must have their academic degrees validated as equivalent to the degree conferred by a regionally accredited college or university in the U.S. The National Association of Credential Evaluation Services (NACES) is a referral resource for applicants who need to have their credentials certified as equivalent. - Considerable knowledge of social work principles, techniques, and practices and their application to individual casework, group work and community problems.
- Knowledge of the psychosocial, socioeconomic and behavioral problems and their treatment.
- Knowledge of governmental and private organizations and community resources.
- Knowledge of laws, regulations and policies which govern the program. General knowledge of medical terminology, disease processes and their treatment is required in certain programs or settings.
- Skill in establishing rapport with a client and applying techniques of assessing psychosocial. behavioral, and psychological aspects of client's problem.
- Ability to establish and maintain effective working relationships with administrative superiors members of case load and their families, and with civics medical, social and religious organizations.
- Ability to train or instruct lower-level social workers, students, or interns.
- Ability to express Ideas clearly and concisely and to plan and execute work.
- Knowledge of available community resources.
- Knowledge of socioeconomic issues impacting the community, children and families.
Must have or obtain and maintain a valid North Carolina driver's license with an acceptable driving record. Background check and negative drug test are required.
