Position Summary
The LCSW position provides evaluation of social and family situations to determine social, family, financial and environmental needs, developing and appropriate and individual social work treatment plan. The LCSW also provides, community resource information, cognitive testing and collaborative team intervention for the best possible patient outcome.
Primary Position Responsibilities
1. Evaluation of social and family situations to determine social, family, financial and environmental needs, with the development of an appropriate social work treatment plan. There is interpretation of social, emotional and family problems to physicians, team members and other caregivers. Completes a report on the above observations which becomes part of the geriatrician’s report.
2. Assessment of the need for therapeutic intervention and accurate recognition of acute situations crisis and applies immediate, time-limited crisis intervention techniques that provide emotional relief and stability.
3. Refers to other counseling resources for issues or needs requiring more in depth or specialized care as needed. Works with other HHS and community programs to problem solve, resulting in improved relationships and satisfactory solutions.
4. Does most Cognitive testing on inpatient consultations prior to the geriatrician’s visit. Will supervise and reassure all memory patients on the computer cognitive testing.
5. Completes assessment intakes, schedules appointments and brings any problem issues to the Director when they arise. Staff is kept up to date regarding scheduling of appointments at the monthly staff meeting. Meets with Director on a regular basis to discuss patients, families and issues that may need a team approach.
6. Regarding billing: Will Review listing of all patients seen previous week and check for accuracy of billing, looking for multiple encounters on some patients, procedures billed properly, and all encounters showing correct providers.
7. Communicates with CBO biller for all necessary corrections, and with the IT department for NexGen technical issues. Attends all super-user seminars and programs to address up-grades in NexGen.
