Salem Transitional Care is looking for a compassionate, organized, and dedicated Social Services Manager to join our team! In this role, you'll serve as an advocate for our residents, helping ensure they receive the support, resources, and person-centered care they deserve. If you're passionate about making a meaningful difference in people's lives while working alongside an interdisciplinary healthcare team, we'd love to hear from you.

What You'll Do

As our Social Services Manager, you will:

  • Advocate for residents and protect their rights, dignity, and well-being.
  • Complete psychosocial assessments and develop individualized care plans.
  • Provide support and guidance to residents and their families throughout their stay.
  • Coordinate referrals to community resources and outside agencies.
  • Participate in interdisciplinary care conferences and collaborate with nursing, therapy, physicians, and other departments.
  • Assist with discharge planning to help ensure smooth transitions to the next level of care.
  • Maintain accurate clinical documentation and social service records.
  • Help identify and address residents' behavioral, emotional, and psychosocial needs.
  • Build meaningful relationships with residents, families, and community partners.

Requirements:

Education

  • Bachelor's or Master's degree in Social Work.

Licenses/Certifications

  • Current LCSW, LSW, or MSW license required.
  • Valid driver's license required.

Experience

  • Experience in a post-acute care or skilled nursing facility preferred.

Knowledge, Skills & Abilities

  • Excellent verbal and written communication skills.
  • Strong organizational and time management skills.
  • Ability to build positive relationships with residents, families, healthcare providers, and community agencies.
  • Ability to work collaboratively as part of an interdisciplinary team.
  • Compassionate, professional, and resident-focused approach to care.
