Summary:
Vighter is recruiting Registered Nurse (RN) Case Managers to provide patient care and nursing administrative support for outpatients clinical services at Joint Base Andrews, in accordance with established policies, procedures, protocols, and guidelines, while supporting the healthcare needs of Service Members, their families, and Veterans.

Principal Accountabilities:
- Participate in all phases of the Case Management Program (CMP) and follow established standards of care.
- Assess patients, develop care plans, coordinate services, monitor progress, and evaluate outcomes.
- Provide case management across inpatient, outpatient, onsite, telephonic and community settings.
- Develop and use case management tools, guidelines, software, databases and community resources.
- Coordinate case management, utilization management, nursing, and social work services.
- Maintain relationships with community agencies and healthcare resources.
- Accurately collect, document, and maintain patient care information.
- Develop individualized treatment plans addressing medical, preventive, rehabilitative, and psychosocial needs.
- Coordinate discharge planning, follow-up care, referrals, and continuity of care.
- Assist Service Members, Veterans, families, and caregivers with complex healthcare needs and available services.
- Coordinate care among facilities, community resources and multidisciplinary healthcare teams.
- Advocate for patients and provide education to patients and families/caregivers.
- Monitor treatment plans, patient progress, healthcare outcomes, and use of resources.
- Participate in interdisciplinary, care coordination, and facility meetings.
- Complete required medical record documentation, coding, tracking logs, and reports.
- Follow DoD, MTF, case management, quality, safety and infection-control requirements.
- Complete required orientation, electronic medical record, and Case Management training.
- Assist with staff training and participate on committees, work groups, or task forces as required.
- Travel off-site when required to support patient care and evaluate appropriate living or recovery environments.
- Follow HIPAA, DoD, DHA, MTF, The Joint Commission and professional nursing standards.
Knowledge and Skills:
- Strong case management and care coordination skills.
- Proficient with Microsoft Office, Outlook, and electronic medical records.
- Strong oral and written communication skills.
- Effective critical-thinking and problem-solving skills.
- Ability to manage complex healthcare, social, and financial patient needs.
- Strong organizational and interpersonal skills when working with patients, families, and healthcare teams.
Education:
- Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE).
Qualifications:
- Minimum of three (3) years of experience for AND or two (2) years of experience for BSN full-time experience in nursing and care coordination, working with children, families, seniors, or groups.
- Maintain an active, valid, current, and unrestricted license (with no limitations, stipulations or pending adverse actions) to practice nursing as a registered nurse in any US state/jurisdiction.
- Current Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) are both required.
- Certified Case Manager (CCM) through the Commission for Case Manager Certification, or certification through the American Nurses Credentialing Center (ANCC); OR certification-eligible with above-designated experience.
Duty Hours / On-call Requirements:
- The expected duty hours for this position are 8 hours per day and 5 days per week. However, this position supports 24/7 operations which may require work to be conducted at night and on weekends.

Travel:
- As required to satisfy the contractual obligations. Commuting is non-compensatory. All official travel is government funded.

Physical Requirements:
- Must be able to stand or sit for more than 8 hours per day.
- Must be able to use an elevator or walk up & down stairs.
- Must be able to lift up to 40 pounds and carry up to 10 feet.
- Must be able to routinely grasp or handle objects, use finger dexterity, bend elbows/knees and reach above and below shoulders.
- Must be able to read and interpret handwritten and typewritten print.
- Must be able to communicate by voice and detect sound by ear.
- Must be able to perform duties in an indoor office or outdoor environment.
- Must be able to pass a criminal background investigation.
- Must undergo Urine Drug Screening (UDS).
- Must be a Citizen of the United States or legally authorized to work in the United States of America.
