Wellness Specialist – Living in Place Program as JASA

As a Wellness Specialist, you will play a critical role in providing targeted care coordination and evidence-based interventions for older adults and their families through JASA’s Living in Place program. Working under the supervision of the Program Director and funded by the NYC Department of Health and Mental Hygiene (DoHMH), you will deliver time-limited care management across home, office, and community settings to ensure seniors successfully connect to life-essential resources.

📝 What You’ll Be Doing

You will balance direct clinical care management with community outreach, benefit advocacy, and structured service tracking.

Key Responsibilities:

  • Care Coordination & Assessment: Conduct comprehensive client assessments; develop, implement, and maintain individualized service plans in home, office, and community environments.

  • Time-Limited Interventions: Provide targeted care management and monitor progress in strict compliance with Living in Place program standards and timelines.

  • Benefits & Entitlements Advocacy: Screen for eligibility, assist with applications, and advocate to secure essential benefits (Medicare, Medicaid, public assistance, etc.).

  • Service Arrangement: Direct-connect clients to transportation, homecare, home-delivered meals, legal aid, housing, and emergency medical or psychiatric care.

  • Structured Outreach: Develop and execute community outreach plans, complete with project goals and progress measurements.

  • Agency Representation: Represent and interpret JASA's mission at community conferences, interagency meetings, and local events.

  • Reporting & Compliance: Complete all required electronic case records, reports, and statistics in compliance with JASA and DoHMH guidelines; inform supervisors immediately of crisis situations.

🎓 What You’ll Need

  • Education: Graduate of an accredited college or university with a Bachelor’s degree in Social Work (BSW) or Psychology.

  • Experience: Minimum of two years of professional human services experience required.

  • Communication: Ability to read, write, and speak English; bilingual Spanish fluency is preferred.

  • Technical Skills: Computer literacy required, including the ability to master program-specific record-keeping software.

  • Travel & Physical Requirements: Ability to travel to clients' homes and negotiate steps or physical barriers in various residences.
