About This Position
*Please note this position does require some weekend coverage and acute clinical experience is preferred.
The Care Manager will:
Coordinate and manage the care throughout the patient's acute inpatient episode of illness; ensure continuity of services and appropriate utilization, as well as management of resources.
Demonstrate leadership abilities by exhibiting a proactive approach to the care management process. Demonstrate a commitment to group goals and teamwork. Assumes responsibility and accountability for your own duties and performance. Participate in the health system's quality improvement initiatives, which include facilitation and implementation of disease management processes, performance improvement, quality and outcome management.
Case management processes and key elements involved include planning, facilitating, advocating, educating, monitoring, evaluating and measuring the various outcomes for each identified patient population.
QUALIFICATIONS: LPN or RN
