Reports To: Home Manager

Position Summary:

  • To significantly & tangibly contribute to providing all the individuals who choose Angels’ Place supports Christ-centered service delivery that fosters health, security, independence, dignity, community integration & spiritual growth in compliance with all person-centered plans & all applicable federal, state & local statutes & organizational standards. Employee judgment & decision-making must align themselves to the organizational mission, philosophy & culture in concept & spirit including interaction with residents, families, co-workers & all others associated with the organization.

Essential Functions:

  • Implement & document all person-centered plans within the philosophy & mission of Angels' Place.
  • Provide all individuals residing within the home with a safe & healthy environment, which fosters independence & age-appropriate functioning both in & out of the home.
  • Provide transportation & appropriate supervision to work, medical appointments, shopping, church, & any other relevant & requested locations & events within the community.
  • Attend & actively participate in residential Spiritual Development gatherings in the home as scheduled.
  • Facilitate daily prayer within the home as assigned.
  • Ensure that all individuals living in the home have available clean, well-fitting, stylish, repaired, age & event appropriate clothing, personal supplies & other possessions that are valued by the individual.
  • Follow all positive behavior intervention guidelines for all individuals.
  • Work with all individuals at home & in community on daily living & leisure skills & function as a positive role model with the goal of increased socialization, integration, inclusion, safety & individualization.
  • Work as a team with co-workers, supervisors, volunteers, administrators, & families to provide support & service delivery of the highest caliber & in line with the organizational philosophy.
  • Consistently complete accurate & timely documentation of progress, status, & needs of each individual living in the home as required and/or requested by Angels' Place, the Department of Community Mental Health, Adult Foster Care Licensing regulations, funding agencies, & other applicable entities.
  • Follow all environmental safety guidelines in the home as required by Angels' Place, AFC Licensing Rules & Regulations, CMHs, & other agencies as appropriate.
  • Participate in & document all monthly environmental safety checks & drills as assigned. (Fire drill, tornado drill, freezer temperature, refrigerator temperature, hot water temperature, etc.)
  • Maintain the home environment, both interior & exterior, that is attractive, neat, & clean & in alignment with the standards accepted by Angels' Place including housekeeping, deep cleaning & ensuring the porches & walkway debris & snow free.
  • Provide consistent feedback to supervisors to ensure an adequate inventory supply is maintained at all times. (Household supplies, food, cleaning supplies, etc.)
  • Consistently complete tasks within the home with the goal of meeting guidelines established by AFC Licensing, CMH, & other applicable agencies.
  • Record all maintenance needs of the home (interior & exterior) in the maintenance log & report all vehicle maintenance needs to the home manager.
  • Assist the individuals living in the home with nutritious meal planning & preparation.
  • Implement individual medical plans as prescribed by their physicians & in compliance with the standards established by Angels' Place, AFC Licensing Rules & Regulations, CMH, including frequency of appointments, medical & medication follow up, medication administration, special diets, etc. Report & record all medical needs & symptoms accurately & promptly.
  • Perform all other position related tasks as assigned.

Position Qualifications:

  • Education: Required: High school diploma/ GED
  • Experience: Required: 2+ years working with Individuals with Developmental Disabilities
  • Physical: Must be able to stand & work without physical restriction in a group home setting for 8 hours/ day (some team lifting may be required). Staff should be able to lift 50 pounds. Some outside work should be expected.

Other Requirements, must:

  • Pass physical, drug screen, TB test, Michigan criminal background check & FBI fingerprinting (all paid by Angels' Place)
  • Must possess a valid Michigan driver's license and maintain a responsible driving record
  • Must agree to comply with all Angels’ Place policies and procedures located in the Employee Handbook, Operations Manual and those required by state licensing agencies, County Community Mental Health agencies, Offices of Recipient Rights and other applicable regulatory agencies.

Work Environment

  • 8 hour shifts, some weekends & holidays required.
