We are seeking a compassionate, dependable, and clinically skilled Licensed Practical Nurse (LPN) to join our personal care team. This position is ideal for a nurse who is passionate about providing exceptional care to seniors living with dementia and other memory-related conditions. As a Memory Care LPN, you will play a vital role in supporting residents' physical, emotional, and behavioral needs while promoting dignity, comfort, and quality of life. The successful candidate will possess strong leadership skills, patience, empathy, and the ability to effectively communicate with residents, families, and interdisciplinary team members.
Responsibilities Include:
*Administering medications and treatments in accordance with physician orders and facility policies
*Monitoring resident health status and reporting changes in condition to the appropriate provider
*Completing documentation and care plan updates accurately and timely
*Supervising and supporting PCA and caregiving staff to ensure quality resident care
*Assisting with coordination of resident appointments, follow-up care, and physician communications
*Responding appropriately to resident behaviors associated with dementia while utilizing redirection and person-centered care techniques
*Providing support and education to residents and family members
*Maintaining compliance with all state regulations, infection control practices, and facility policies
*Collaborating with all departments to ensure residents' medical, social, and emotional needs are met
Qualifications:
*Current New York State LPN license in good standing
*Strong organizational, communication, and leadership skills
*Ability to work independently and make sound clinical decisions
*Experience in assisted living, long-term care, geriatrics, or memory care preferred
*Understanding of dementia care, behavior management, and person-centered care practices preferred
*Compassionate, patient, and committed to enhancing the lives of seniors
