Nurse (Part-Time)

Position Overview

This position is responsible for providing nursing services to children and youth, medical consultation to FCCP staff and foster families, and following the mandated and recommended health services activities needed to support optimal health for children in foster care. Nurses must comply with Early and Periodic Screening, Diagnostic and Treatment (EPSDT) standards and state regulations while performing functions noted below.

Key Responsibilities

  • Conduct comprehensive health assessments, including HIV and intake assessments, first aid, triage, substance abuse screenings, developmental and mental health evaluations, and physical examinations for signs of abuse.
  • Provide health education and training to children, caregivers, and staff, including reproductive health education and wellness promotion.
  • Educate and support caregivers by monitoring healthcare needs and participating in home visits to ensure continuity of care.
  • Administer and manage medications in accordance with established policies and procedures.
  • Coordinate community healthcare services, facilitate medical appointments, and ensure appropriate follow-up for all health-related needs.
  • Serve as a liaison between the voluntary agency and Local Department of Social Services (LDSS), ensuring timely communication regarding children's health concerns, safety, and well-being.
  • Develop individualized treatment goals based on assessments, screenings, medical history, and collaboration with an interdisciplinary team.
  • Monitor and maintain accurate documentation within the CONNECTIONS Health Services Module, ensuring records are timely, comprehensive, organized, and compliant with regulatory standards.
  • Maintain health records and files to support continuity of care, oversight, and quality assurance requirements.
  • Implement infection control protocols and ensure adequate nursing coverage, medical supplies, and equipment availability.
  • Provide 24/7 on-call support for urgent health concerns involving children and youth and respond to inquiries from State, City, and County authorities as needed.
  • Collaborate with multidisciplinary teams to address the physical, developmental, behavioral, and emotional health needs of children and families.
  • Perform additional duties and special projects as assigned by management.

Qualifications

  • Satisfactory completion of Mandated Reporter Training.
  • Licensed Practical Nurse or Registered Nurse required.
  • First Aid and CPR certification required.
  • Obtain satisfactory background checks pursuant to NY Health Care Facility requirements.
  • Must be at least 21 years of age.
  • Valid State Driver's License required.
