Description:

The CSP-HI Case Management Project Coordinator is a dual-impact role combining expert case management with program coordination and administrative leadership. The CSP-HI funding stream through MassHealth supports housing stabilization and wraparound supportive services for individuals transitioning from chronic homelessness into permanent housing. As Victory Programs expands CSP-HI services across multiple programs, this role will play a critical part in developing and coordinating the agency’s CSP-HI operations.

The Coordinator is responsible for key program functions including outreach, referral management, client enrollment, compliance tracking, interdepartmental coordination, and support of ongoing program implementation. Collaboration is central to this role, as the Coordinator will work closely with multiple agency programs within Victory Programs to identify eligible clients, facilitate seamless service delivery, and ensure continuity of care.

In addition to program coordination responsibilities, this role maintains a dedicated caseload and provides comprehensive, client-centered case management to individuals navigating housing transitions, chronic health conditions, substance use, and mental health challenges. Services are delivered using a Housing First and Harm Reduction approach, supporting clients from housing placement through long-term tenancy stabilization.

Reporting to the Director of the Community Based Housing Division, the CSP-HI Case Management Project Coordinator will help drive service quality, program development, compliance, and cross-program collaboration while supporting the continued growth of CSP-HI services throughout the agency.

Location: 434 Massachusetts Ave. Boston, MA (Hybrid Eligible after 90 days)

Essential Functions:

Project Coordination

  • Coordinate day-to-day implementation of CSP-HI-funded program activities
  • Ensure compliance with all funding requirements, reporting deadlines, and documentation standards
  • Track program deliverables, outputs, and outcomes in alignment with funder expectations
  • Maintain organized systems for monitoring project milestones and required submissions
  • Serve as primary liaison regarding program operations
  • Support preparation of program reports, audits, site visits, and funder communications
  • Monitor data quality and ensure timely and accurate entry into required systems

Case Management Coordination

  • Provide coordination support to case management staff to ensure consistency in service delivery
  • Prioritize intraagency collaboration: work closely with VPI programs to ensure clients receive uninterrupted services when transitioning from temporary shelter or programs to permanent housing and ongoing stabilization supports through CSP-HI.
  • Assist in developing tools that support case managers
  • Support onboarding and training of case managers in program procedures and documentation
  • Provide ongoing training and coaching to staff, focusing on CSP-HI stabilization standards.
  • Monitor caseload assignments to ensure equitable distribution and timely service delivery.
  • Facilitate monthly internal agency team meetings and represent the program at state and partner CSP-HI meetings.

Direct Case Management & Housing Support

  • Maintain a direct caseload of clients at various stages of the housing continuum.
  • Assist clients with program enrollment, insurance navigation, and accessing primary/specialist healthcare.
  • Conduct strengths-based assessments to develop concrete, measurable, and time-limited service plans.
  • Support clients through the processes of lease-up, move-in, and housing stabilization.
  • Conduct regular home visits and community outreach to monitor housing stability, identify early warning signs of crisis, and provide immediate intervention.
  • Support clients with independent living skills, including budgeting, lease compliance, neighborhood engagement and household management.
  • Act as a liaison between internal/external housing providers, mental health services, and substance use treatment programs.

Non-Essential Functions:

  • Represent the program in the community and to designated partners and funding entities.
  • Identify alternative funding sources supportive of the program mission.
  • Perform other duties as deemed necessary by your supervisor or their designee.

Essential Qualifications:

  • Education/Experience (One of the following):
    • Bachelor’s degree in Social Work, Psychology, Public Health, or a related field.
    • OR Lived experience with homelessness, mental health, recovery, or chronic health management.
    • OR Minimum of 2 years of experience in human services (homelessness, mental health, or substance use).
  • Deep understanding of Housing First, Harm Reduction, and Trauma-Informed Care.
  • Experience managing multiple data systems.
  • Extensive experience building effective relationships and coordinating services across multiple programs.
  • Proven crisis intervention, de-escalation, and problem-solving abilities.
  • Reliable transportation for community-based outreach
  • Proficiency in MS Office and Google Workspace.
  • CPR Certification and TB Screening (both can be obtained upon hire).

Preferred Qualifications:

  • Master’s degree in Social Work (MSW) or a related field.
  • Experience supervising staff and/or interns.
  • Certification in Peer Support, Recovery Coaching, or Community Health.
  • Fluency in Spanish or Haitian Creole.
  • A valid Massachusetts driver’s license with a safety driving points of 00, 99, 01, 02, or 03.

Position Type: Full-Time (40 Hours); Exempt

Rate: $58,520 Annually

Victory Programs offers a generous paid time off package to employees working 20+ hours a week including vacation, sick, and personal time as well as holidays. Upon hire, employees working 20+ hours a week are entitled to up to: 4 personal days, 160 sick hours, 10 holidays and 3 1/2 weeks of vacation. These figures adjust based on full time or part time status and allocated vacation time increases with length of employment.
