DESCRIPTION OF POSITION

POSITION SUMMARY

The MDS Coordinator is a registered nurse (RN) or licensed vocational nurse (LVN) who has developed managerial and clinical skills through experience and education. She/he has the ability to coordinate and organize nursing care related to the resident assessment, as well as conform to Medicaid/Medicare guidelines. She/he is directly responsible to the Director of Nursing.

Requirements:

POSITION QUALIFICATIONS

EDUCATION:

1. Graduation from an accredited registered (RN) or vocational (LVN) nursing program.

2. Progressive experience in nursing practice and nursing leadership.

3. Current license to practice as a registered nurse (RN) or licensed vocational nurse (LVN) in California.

4. Experience in nursing practice including 2 years’ experience in long term care nursing.

DUTIES AND RESPONSIBILITIES:

1. Coordinates gathers data and completes Resident Assessment Instrument in an accurate and timely manner.

2. Implements individual plans of care with interdisciplinary approach in accordance with federal, state and local regulations and the established policies and procedures of Coalinga Regional Medical Center.

3. Transmit, or oversees this function, of MDS data to state agency adhering to federal and state guidelines.

4. Evaluates and re-evaluates as necessary resident needs and level of care by the standards and requirements of licensure and regulations and MDS requirements.

5. Coordinates efforts from all resources available, i.e., clients, staff, health care professionals and the care plan team to achieve optimal plan for each individual plan of care.

6. Participates in the processing of Medicaid/Medicare forms and assists in the processing of additional insurance including VA required forms.

7. Attends and participates in individual resident care planning meetings, quarterly QI/QA committee meetings and other committee meetings as deemed appropriate by the DON.

8. Assures that all assessments are completed in a timely manner by all departments and review for completeness and accuracy of information.

9. Participates and assists in the overseeing of the care planning and modifying of care planning process.

10. Monitor the quality and appropriateness of nursing care.

11. Participates in the developments, periodic review and evaluation of nursing policies and procedures.

12. Participates in surveys by authorized government agencies.

13. Maintains privacy and confidentiality of records, conditions and other information relating to residents, other employees and the facility.

14. Attends and participates in staff on-going training and educational classes.

15. Coordinates wit therapies-Occupational, Physical and Speech-regarding resident rehabilitation.

16. Other duties as assigned by the Director of Nursing including, but not limited to, on call duty and other supervisory duties.

17. FOLLOWS SAFETY, INFECTION CONTROL, AND OTHER GUIDELINES AS REQUIRED

18.PROMOTES AN ETHIC OF CONTINUOUS QUALITY IMPROVEMENT IN ALL MATTERS ASSOCIATED WITH THE POSITION, AND SHALL DOCUMENT SPECIFIC QUALITY ASSURANCE IMPLEMENTATION

19. PERFORMS ALL OTHER TASKS AND DUTIES AS MAY BE ASSIGNED

20. DRESSES AND PRESENTS SELF IN A PROFESSIONAL MANNER AT ALL TIMES. ENTIRE DEPARTMENT FOLLOWS COALINGA REGIONAL MEDICAL CENTER APPEARANCE POLICY

21. WEARS CORPORATION ISSUED NAMETAG AT ALL TIMES

22. CONDUCTS SELF ETHICALLY AT ALL TIMES.
