The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.
Job Overview:
We are seeking a Part Time Social Worker LCSW in our Monmouth Medical Center located in Lakewood. The LCSW will provide direct clinical social work services to patients and their families in an interdisciplinary team approach.
Qualifications:
Required:
- Bachelor’s degree in Social Work or related field.
- LCSW license in NJ required.
- Minimum 1 year of experience, preferably in bereavement and emotional guidance.
Scheduling Requirements:
- Part-Time (28 Hours per week, 3 days a week)
- Day Shift: 8:00am – 4:30pm
- Must be willing to travel to Monmouth Medical Center in Long Branch 1 day per week.
Essential Functions:
- Complete initial psycho-social assessments on assigned patients.
- Provide psychotherapeutic counseling with individuals, family or groups, giving clinical consultations as needed.
- Providing patient centered advocacy.
- Collaborate with interdisciplinary team, managing complex discharge plans.
- Building and maintaining relationships with internal/external resources.
- Make referrals to community agencies and entitlement programs and continuing ongoing education.
- Ensure continuity of care for patients with complex post-acute needs and maximizes the patients and families’ abilities to meet the continuing needs within their social and financial framework.
- Understands of all stages of growth and development to assess each patient’s age specific needs to provide individual treatment and care.
- Provide bereavement and grief counseling to elderly patients, families and caregivers following illness, loss, or life transition.
- Offer supportive coping strategies and emotional guidance during difficult periods.
- Connect seniors and families with appropriate resources and educate families on advance care planning.
- Maintain accurate documentation to ensure compliance with organization standards.
- Outreach initiatives and community education related to aging and grief support.
- Educate families on Advance care planning.
