POSITION SUMMARY:

In coordination and collaboration with the assigned Clinical Team Manager, is responsible for providing skilled nursing care to patients and their families. Coordinates care with other members of the home care team; supervises Home Health Aides. The majority of patient visit time is devoted to patient evaluations, and informational visits.

AREAS OF RESPONSIBILITY:

• 1. Observes and reports any signs or symptoms indicative of changes in condition of the patient and family situation to the facility supervisor and documents such reports and observation in the clinical record.
• 2. Performs assigned nursing procedures, including the administration of treatments and medications in the care of the patient, in accordance with the State Nurse Practice Act and Plan of Care.
• 3. Prepares equipment and materials for treatments observing aseptic technique as required.
• 4. Assists with the rehabilitation of patients according to the patient care plan.
• 5. Maintains confidentiality of medical records.
• 6. Accurately documents all events related to patients and completes all related documents as required by state and/or federal requirements.
• 7. Notifies the team leader of changes in the patient’s condition and documents appropriately.
• 8. Participates in in-service education programs as required by state law.
• 9. Completes appropriate documentation with all patient care including, but not limited to updates.
• 10. Consistently demonstrates competency with technical nursing skill according to the legal scope of practice.
• 11. Under the direction of a Registered Nurse, may perform supervisory function in homecare only under programs managed through First Choice Medical Staffing.
• 12. Performs other related duties and responsibilities as deemed necessary.
