Position Overview:
The Housing Case Manager is responsible for determining service plan goals, targets, and action steps for clients.
Essential Duties & Responsibilities:
- Meeting with clients in-person or virtually to conduct an intake assessment that includes collection of information including demographic, financial, and social services information
- Upon completion of the intake assessment, the HCM will document all assessments accurately and will ensure the payer approves the services
- Following the initial assessment, the HCM will also be responsible for the case management of pre-tenancy services, tenancy-sustaining services, and for the identification of home modification needs
Required Skills and Qualifications:
- Associate's degree in human services or related field
- 1 year of experience providing case management services focused on pre-tenancy and tenancy-sustaining services
- Must have a reliable telephone
- Must be empathetic to the needs of a diverse socio-economic population
- Strong interpersonal skills
- Knowledge of community resources, state and federal benefits and services
- Strong project management skills with the ability to administer multiple cases simultaneously and prioritize workload.
- Basic math and budgeting skills
- Ability to work effectively as a team member with other members of program and agency leaders
- Demonstrated prior success in case management functions or eligibility determinations for community-based social service organizations
- Problem-solving abilities
- Ability to communicate and collaborate with health providers and community partners
- Strong internet, database and computer skills
- Good documentation skills
- Ability to read and write Spanish preferred
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer
- Must be able to lift up to 15 pounds at times

Compensation:
A salary within the range of $24.51 to $33.17 per hour will be provided to the successful candidate having met the minimum requirements for the position consistent with the MOC compensation philosophy.
