Description/Position Summary
The Licensed Therapist (LICSW, LPC, or LMFT) is responsible for providing compassionate, evidence-based mental health services to clients within long-term care settings. This position plays a vital role in supporting the emotional, psychological, and behavioral well-being of individuals by delivering therapeutic interventions and collaborating with interdisciplinary teams to ensure coordinated, person-centered care.
Essential Responsibilities
--Conduct comprehensive initial assessments and develop individualized treatment plans.
--Provide individual therapy sessions and occasional family therapy using clinically sound, evidence-based approaches.
--Deliver therapy services within nursing homes and assisted living facilities.
--Support clients experiencing a wide range of mental health concerns including depression, anxiety, medical issues, severe mental illness, substance abuse history, trauma, grief, and adjustment issues.
--Maintain timely, accurate, and confidential documentation in the electronic medical record (EMR) via SOAP note format, in accordance with HIPAA and company policy.
--Collaborate frequently with psychiatric providers, nursing staff, and facility leadership to ensure continuity of care.
--Participate in monthly virtual team meetings and case consultations.
--Provide crisis intervention and follow-up as needed to ensure client safety and stability.
--Monitor client progress and adjust treatment plan goals and interventions as clinically indicated.
--Engage in ongoing professional development and continuing education to maintain licensure and enhance clinical practice.

Requirements:Physical & Environmental Demands
--Ability to travel to assigned facilities as needed.
--Ability to sit, stand, and move throughout the workday, including light lifting (<25
lbs).
