About This Position

Description:

The Physical Therapist provides skilled physical therapy services to patients in their homes in accordance with physician orders and established care plans. This role includes performing comprehensive assessments, developing and implementing treatment plans, and collaborating with the interdisciplinary care team to support patient recovery, mobility, and independence. The Physical Therapist is responsible for timely documentation, patient and caregiver education, and ensuring compliance with all agency policies and regulatory standards.

Essential Duties and Responsibilities
  • Perform initial and ongoing comprehensive patient assessments, including OASIS assessments.
  • Develop, implement, and revise treatment plans in collaboration with the physician.
  • Assist in the development and implementation of the interdisciplinary patient care plan.
  • Evaluate patients using diagnostic and functional ability tests.
  • Provide therapeutic interventions to relieve pain and restore function.
  • Educate patients and families on therapy programs and safe use of mobility devices.
  • Instruct patients in the use of wheelchairs, braces, crutches, canes, prosthetics, and orthotics.
  • Coordinate outpatient services when needed.
  • Document all evaluations, treatments, and patient responses in a timely manner.
  • Communicate patient status changes to the physician at least every 30 days or as needed.
  • Collaborate with the interdisciplinary team, including Home Health Aides.
  • Supervise Physical Therapy Assistants and paraprofessionals per agency policy.
  • Participate in case conferences, staff meetings, and in-service training.
  • Complete clinical/progress notes within 48 hours of each visit.
  • Assist with patient admissions, including evaluations and care planning.
  • Participate in discharge planning and complete therapy discharge summaries.
Work Environment and Physical Demands
  • Must be able to travel to patient homes, which may not be wheelchair accessible.
  • Requires sufficient hearing, vision, and dexterity to perform patient assessments and treatments.
  • May be required to bend, stoop, reach, and assist with patient weight up to 250 pounds.
  • May lift or carry up to 30 pounds.
Confidentiality

Employee will have access to patient medical records and must maintain strict confidentiality in accordance with HIPAA and agency policies.

Requirements: Job Requirements
  • Graduate of a Physical Therapist program accredited by CAPTE.
  • Current Physical Therapist license in the State of Indiana.
  • Minimum one (1) year of institutional or home health experience preferred.
  • Current CPR certification.
  • Successful completion of a criminal background check.
  • Valid driver’s license, reliable transportation, and ability to travel to patient homes.
  • Ability to perform physical tasks including bending, stooping, reaching, lifting, and assisting with patient mobility.
  • Effective verbal and written communication skills in English.
