Pharmacist - VBC and Population Health at LifeBridge Health – Baltimore, Maryland

The Pharmacist, Value-Based Care & Population Health is a system‑level clinical pharmacist who supports value‑based care initiatives through medication optimization, quality improvement, and interdisciplinary collaboration. This role focuses on improving clinical outcomes, reducing avoidable utilization, and supporting high‑quality, cost‑effective care across population health and risk‑based programs.The Pharmacist works collaboratively with the Senior Lead Pharmacist and interdisciplinary care teams to execute pharmacy‑led initiatives aligned with system priorities.

QUALIFICATIONS: PharmB or MScPhm or PharmD

PGY-1 + PGY-2 residency (Ambulatory Care Residency Preferred) or PGY-1 residency plus 2 additional years of relevant direct patient care experience, or four post-graduate practice years of relevant direct patient care experience + board certification

Pharmacist License - Current Maryland license or eligibility to obtain Maryland license
