Job Summary:
As a Psychogeriatric Case Manager, you will be part of a team that co-ordinates a support network and linkages for older adult and seniors with serious mental illness and other challenges (e.g., addictions). A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity, and self-respect. The role will require direct service and support to clients in area such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.

Crisis Outreach Services for Seniors (COSS) is an on-call mobile crisis service for older adults (65+ years) who are frail, socially isolated, at-risk of repeated hospital use and/or elder abuse due to living with mental health and substance use challenges. The COSS team delivers services within 24-48 hours either in the individuals own home or in their local community. Short-term crisis response is available 365 days a year. The team provides crisis counselling, short-term intensive case management, harm reduction, mental health and addictions support, health assessment and care as need by a Nurse Practitioner, and referral to primary care. COSS is a partnership with Woodgreen and Reconnect ensuring that all of Toronto has access to this service. LOFT’s COSS team supports those individuals living within the northern Toronto geographical area.

What You Will Do:

• Assists persons served in the development and implementation of a care plan tailored to the needs of the person served and issues designed to improve quality of life and assist in the mitigation of responsive behaviours. Identify and refer to resources specific to those needs and issues;
• Act as a liaison and advocate for the person served among those involved with the individual’s care including family, other care-partners, medical and other multi-disciplinary providers;
• Complete assessment such as RAI and OCAN and provide timely and accurate record keeping and reports as required;
• Provide supportive counselling or referral and linkages to health care, programs or service providers;
• Provide crisis management/intervention and assist in the development of crisis plans for persons served;
• Develop respectful and trusting relationships with persons served using Recovery and Psychosocial Rehabilitation principles;
• Maintain strict confidentiality and follow all other required policies and procedures;
• Consistently interact with persons served, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization and the Behavioural Supports Ontario Project;
• Effectively participate as a member of the team; attend staff/team meetings, care conferences for person served, other multi-disciplinary meetings, committee meetings etc. as required;
• Attend and actively participate in all required training;
• Liaise, collaborate and coordinate with other professionals, service providers and partners within the Behavioural Support Services
• Complete other duties, from time to time, as assigned by the Program Manager and/or Associate Director.

What You Bring:

• Completion of a diploma/degree in Social Services/Human Services or equivalent;
• Minimum three years related experience in providing support to vulnerable seniors and older adults with responsive behaviours related to mental illness, addictions, dementia or other neurological conditions;
• Group counselling related to mental health, addictions, and related issues.
• Knowledge of community resources and support
• An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with client on goals/priorities.
• Crisis intervention skills, with well-developed problem solving abilities and interpersonal skills
• Good oral and written communication skills and computer literacy, including MS word and Excel.
• Counselling, responding and assessment skills.
• Creativity and flexibility to approaches.
• Ability to work effectively as part of a team and interact with workers from a variety of agencies.
• Understanding of cultural competency and framework
• Possession of a valid driver’s license, and a reliable vehicle in order to travel is required.
• Completion of Gerontology is an asset.
• Ability to communicate in a different language, an asset.

Work Environment:

• Staff will be required to work in client’s own homes and may therefore be exposed to secondhand smoke.
• Occasional evening and weekend hours may be required.
• Frequent travel will be required using own vehicle.
