Citizens Memorial Hospital (CMH) is hiring a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Medical Home Department in Bolivar, Missouri — someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve.

What Your Day Looks Like: Medical Home strives to improve patient outcomes by facilitating care provided within the Patient Centered Medical Home and providing support through transitions in care.
As a Registered Nurse/License Practical Nurse, you will be:

Providing education and resources on medical diseases, treatments, and medications significant to the individual’s chronic or preventative healthcare needs to the patients and their caregivers.

Working with patients and other members of the healthcare team to set and achieve health goals through motivational interviewing.

Recommending action steps and interventions to assist patients with improving health and reaching identified goals.
Qualifications:
Education: Completion of an accredited program (Licensed Practical Nurse or Registered Nurse)
Experience: 1 year of case management, or 3 years of related healthcare experience.
Skills: Organization, able to work in a fast-paced environment, multitasking, patient care, strong written and verbal communication.
Licensure/Certification: Active Missouri License as a Licensed Practical Nurse or Registered Nurse.
Physical Requirements: Ability to work at a computer 90% of the day and stand for the remaining 10%
