Position Summary

The Transition Coordinator ensures seamless transitions for patients from acute care to home health services, combining office duties with direct patient interaction. This role involves collaborating with discharge planners, nurses, and other healthcare professionals to create and implement individualized transition plans. Key responsibilities include coordinating discharge processes, educating patients and families about home health services, entering patient information, completing insurance authorizations, and providing general clerical support. The Transition Coordinator also obtains necessary information from physician’s offices and other resources.

Minimum Qualifications

• LPN, Medical Assistant, other healthcare experience required.
• MDS experience preferred.
