RN Care Manager in Bolivar, Missouri at Citizens Memorial

Citizens Memorial Hospital (CMH) is recruiting a RN Acute Care Manager to help care of our patients. Join our interdisciplinary hospital team for discharge planning and patient education! Acute Care Managers facilitate the care of our inpatients from admission through discharge in collaboration with the associated care team, ensuring progression of stay and efficient utilization of needed resources individualized to patient needs. In addition to navigating patients through both their expected acute stay and ongoing discharge needs, this nursing role is primarily focused on patient education and validation of patient understanding and promoting quality patient care outcomes. Strong communication and interpersonal relationship development skills are critical. This nursing role is patient-facing, responsible for collaborating and communicating discharge plans daily with the patient, family, and interdisciplinary departments. Involving the patient as an active participant in their discharge planning, taking their preferences into account, and ensuring patient understanding of their responsibilities in managing their health after discharge are key objectives of this unique position.

Job Requirements:

• Education: Associates or Bachelors
• Skills: Clinical expertise, strong decision-making and communication
• License: RN
• Physical: The ability to sit or stand for long periods of time
• Previous clinical experience in patient education and/or an acute care setting is preferred.
