At Lifted Hospice, our mission is simple yet powerful: Helping people live life to the fullest. We serve the communities where we live and work by delivering compassionate, clinically excellent end-of-life care. With a commitment to dignity, comfort, and quality of life, we walk alongside patients and their families—meeting them where they are, when they need it most.
What makes us different is how we show up. We care for patients and families like they’re our own—and we support each other the same way. At Lifted Hospice, you’ll be part of a culture built on trust, unity, and joy. We serve from a place of favor, and we never forget the sacredness of what we do.
A Calling with Impact
At Lifted Hospice, our Clinical Navigators help patients, families, and providers take the first step into hospice care—with clarity, compassion, and confidence.
As a Clinical Navigator, you’ll combine your nursing expertise with strong communication and outreach skills to assess eligibility, provide education, coordinate admissions, and facilitate seamless transitions into hospice care. You are often the first clinical face patients see—bringing reassurance, presence, and peace to what can be an overwhelming moment.
