Position Description
The Case Manager leads the coordination and advocacy functions for participants enrolled in the CHILD Waiver program. This role serves as the primary architect of the Person-Centered Service Plan (PCSP), ensuring that every child’s goals for independence and community integration are met. The Case Manager manages the complexities of the MWMA, facilitates transition planning, and acts as a neutral advocate.
Key Responsibilities
Assistance for everyone can range from occasional verbal reminders to full staff assistance, depending on the individual's functioning level.
- Develop and implement personalized care plans (PCSP) for clients, ensuring goals align with community involvement and independence.
- Coordinate and conduct annual functional assessment reviews within 365 days of the prior assessment.
- Identify, connect, and monitor community-based services that align with each child's needs and preferences.
- Serve as a neutral liaison between clients, families, and service providers, adhering to conflict-free case management standards.
- Utilize the Medicaid Waiver Management Application (MWMA) for all reporting and program integrity documentation.
- Lead the transition of care for adolescents into adult medicine while navigating legalities like supported decision-making.
- Apply "Disability Etiquette" by adapting communication to meet cognitive and sensory needs.
Education & Training and/or Experience:
- Education: Bachelor’s degree in a human services field as required by state regulation.
- System Knowledge: Completion of all state-required MWMA training modules before system access.
- Experience: Proven experience in managing complex behavioral health profiles and neurodivergent sensory needs.
Language/Math/Reasoning Ability:
- Ability to draft professional medical necessity letters for specialized equipment or waiver services.
- Ability to synthesize complex clinical data into clear language for families and support staff.
Licenses, Police Check, Registration, and Certifications:
- Current Valid Kentucky Driver's License and proper vehicle insurance.
- Acceptable background checks from the AOC, KNAR, and Central Registry (CAN).
- Ability to obtain and maintain First Aid/CPR and CORE training.
