At Reema Health, we believe a personalized, community-based approach to care makes it easier for anyone to get what they need to improve their lives. We believe a relationship with a real person, someone who listens, meets members where they are, and understands the community's resources , can ease those burdens.

A note about this role: This position requires an active Kentucky Registered Peer Support Specialist credential; however, the day to day responsibilities are those of Reema’s Community Health Navigator, internally referred to as a Community Guide. This is a community-based health navigation and member engagement role focused on outreach, addressing barriers to care, connecting members to resources, and helping members complete activities that improve their health and wellbeing. Your peer support training and experience will be valuable in building trusted, person-centered relationships with members, but this is broader in scope than a traditional Peer Support Specialist position.

As a Community Health Navigator, you’ll build genuine relationships with members while actively guiding them toward greater engagement in their own healthcare. This is purposeful work, and you’ll partner with members to address social barriers so that each member can accomplish health activities that improve their wellbeing.

You’ll work through Reema’s technology platform daily to reach members, schedule visits and document your work. As a growing startup, roles evolve quickly. We look for people who embrace fluidity as an opportunity to grow and innovate.
