Citizens Memorial Hospital (CMH) is hiring a Social Worker to care for our patients at the Dialysis Clinic in Bolivar, Missouri. CMH is made up of 34 primary and specialty clinics, 6 long-term care facilities, and 1 residential care facility. About the department: Supporting patients of all ages, the Social Services Department is available 24/7 to offer assistance with discharge planning, resource referrals, and consultative services. Working as part of an interdisciplinary team, Social Workers and Designees provide clear communication to patients, family members, and other team members.

A typical day includes:

• Develops comprehensive psychosocial assessments, which examine the presenting problem(s) as well as relevant biopsychosocial and environmental factors. Communicates relevant aspects of the assessment to the patient, family/caregivers, and members of the health care team.
• Responsible for the timely and accurate completion of psychosocial histories for each patient, interviewing patient and/or family and compiling the assessment within 72 hours of admission.
• Provides case management for each patient admitted to the unit by acting as a liaison between the patient and the interdisciplinary team.
• Facilitates group, individual, and family meetings to address psychosocial needs and assistance with adjustment to loss or illness, providing education to patients and families/caregivers about unit philosophy, expectations of patient/family/caregiver involvement in the treatment process, and helping patient/family/caregiver to understand the parameters of the process.

This position is: Full-time, days

Job Requirements:

• Education: MSW or BSW from an accredited institution
• Experience: 1 year of experience in hosptial or long-term care setting preferred
• Skills: Patient care, customer service, problem-solving, listening
• Licensure/Certification: N/A
