Requirements:
  • Resident and Family Engagement: Build positive relationships with residents and families, address general questions about therapy services, and help promote a welcoming rehabilitation experience.
  • Therapy Program Coordination: Collaborate with the Director of Rehabilitation and therapy team to support resident participation, coordinate communication, and identify opportunities to improve the resident experience.
  • Rehabilitation Education: Explain the purpose and general benefits of physical and occupational therapy, encourage participation in prescribed rehabilitation programs, and reinforce education provided by licensed clinicians.
  • Communication and Follow-Up: Serve as a liaison between residents, families, therapy staff, nursing, and facility leadership to help address concerns and facilitate timely communication.
  • Resident Experience: Help identify barriers to therapy participation, communicate resident concerns to the appropriate clinical team members, and support initiatives that improve satisfaction and engagement.
  • Interdisciplinary Collaboration: Participate in appropriate care conferences, resident meetings, and interdisciplinary discussions as requested.
  • Documentation and Reporting: Maintain appropriate records of resident and family interactions, follow-up activities, and other assigned administrative responsibilities in accordance with facility policies.
  • Quality Improvement: Assist with identifying opportunities to improve therapy communication, resident engagement, and coordination between rehabilitation and other departments.
  • Professional Representation: Promote the rehabilitation department's services and value while maintaining professionalism, confidentiality, and compliance with facility policies.

Qualifications

  • Current, valid state license as a Physical Therapist Assistant (PTA) or Occupational Therapy Assistant (COTA/OTA).
  • Experience in skilled nursing, rehabilitation, assisted living, or another healthcare setting preferred.
  • Strong communication, interpersonal, and relationship-building skills.
  • Ability to work collaboratively with residents, families, clinical teams, and facility leadership.
  • Excellent organizational, problem-solving, and follow-up skills.
  • Compassionate, resident-focused approach with a commitment to quality care and service.
  • Ability to maintain confidentiality and comply with applicable healthcare regulations and facility policies.

Scope of Practice

The Resident Care Liaison supports communication, resident engagement, and coordination of rehabilitation services.
