Job Description

We are seeking a compassionate and organized Intake Coordinator to join our team, dedicated to supporting families in accessing Community Living Supports (CLS) and Respite Care services. This role offers an opportunity to make a meaningful difference in the lives of families and individuals by providing exceptional service and guidance throughout the intake process.

Key Responsibilities:

- Serve as the primary point of contact for families seeking CLS and Respite Care services, providing information and support throughout the intake process

- Assess families’ needs and eligibility for available services, ensuring accurate and timely documentation

- Coordinate with service providers, case managers, and community resources to facilitate smooth intake procedures

- Maintain detailed and organized records of all interactions, assessments, and service plans

- Educate families about available programs, eligibility criteria, and application procedures

- Follow up with families to ensure ongoing support and address any questions or concerns

- Stay informed about updates and changes in community resources, policies, and regulations related to CLS and Respite Care

Skills and Qualifications:

- Excellent communication and interpersonal skills, with a compassionate approach to family support

- Strong organizational and time-management abilities

- Experience in social services, healthcare, or related fields preferred

- Knowledge of community resources, eligibility requirements, and service coordination

- Ability to handle sensitive information with confidentiality and professionalism

- Proficiency in computer applications and record-keeping systems

- Ability to work independently and as part of a collaborative team
